It would be nice to allow guests to enter information on a custom form. I currently use jotform.com to send an arrival questionnaire to ask the guests how everything was upon arrival such as cleanliness. I also send them a departure questionnaire/survey. It would be nice to have that integrated in OR.
Great idea! I was looking at using Google Forms for something similar ("If you have a maintenance request, please fill out this form," etc.) but will have to look into Jotform.
However, if this feature is on OR's roadmap, I'll put that project off, so please let us know, OR guys! :)
I was trying to add a simple "Contact us" form, but am not given the option.

I am doing the same now, but with Google forms. It is very annoying since the collected information cannot be imported into OR unless manually. If you have multiple reservations, it becomes an impossible task. It defeats the idea of having a unified picture/spreadsheet of your guests, which you might want to use later in triggered emails or else.
On a similar note, I would like the ability to simple edit existing forms.
For example, I'd like to remove the following highlighted text from the Form "Form: 'Booking Fixup - Step (1) Start Page'", but I'm currently only able to customize the text in the top blue box (and nothing else on this Form):
We used an arrivals form in our previous PMS to record arrival times and wine preferences. This would be SUPER HELPFUL to have it integrated into OR!
